Benefits under a Personal Accident Plan
Accidental Death Benefit – payable for accidental bodily injury if death occurs within twelve calendar months of bodily injury if death occurs within twelve calendar months of bodily injury and is directly or solely attributable to the accidental bodily injury.
Permanent Total Disablement – payable once an Insured becomes disabled permanently, totally and absolutely from engaging in each and every occupation or employment for compensation within twelve calendar months of bodily injury and not followed within twelve calendar months of said bodily injury by death.
Permanent Partial Disablement – payable when injuries incurred accidentally does not result in loss of life but where there is actual severance of a body member or loss of use of such member.
Medical Reimbursement – payable when by reason of injury caused by accidental means, the Insured shall require treatment by a legally qualified physician or surgeon or confinement in a hospital.
